To get your student health insurance card, our plan administrator, Relation Insurance Services, requires that students create an account and then a password. Once the account and password are created, the student would have access to printing their ID card, accessing their 1095-B insurance document, view claims status, and more. The student is responsible for setting up that account with Relation. No one besides the student should know their password for the Relation site. The step by step directions for setting up an account are below:
Step 1: Go to 4studenthealth.com
Step 2: In the “School/Organization” drop down box, please select “Touro University-Nevada”.
Step 3: In the “Select Your Plan” box, please select “Student Health Insurance Plan (SHIP)”.
Step 4: Then click on “View Your Plan”.
Step 5: Make sure that under “Currently Displaying” that “Plan Year 2017-2018” is selected.
Step 6: Scroll down the page until you see the “Helpful Quick Links”, just below that you can select “Insurance ID Card to download and print your ID card”
Step 7: If it is your first time logging into the Relation Student Health Insurance Portal, you will need to click the “Create Account” button. After creating your account you will be able to print your insurance card. If you have already created an account, put your last name, student ID number and birth date into the required fields and hit “Sign In”. The next page will present you with a printable ID card.
